Brazil pharmacy stocks tumbled as Mercado Livre moved into prescription drugs, threatening the chains’ best customers.

Shares in Brazil’s big pharmacy chains tumbled on Thursday after Mercado Livre said it would start selling prescription medicines. Brazil pharmacy stocks closed down as much as 8%, with Pague Menos hit hardest.
The e-commerce group’s São Paulo pharmacy is due to start selling antibiotics and GLP-1 weight-loss pens today, Friday 25 September. For these drugs, the pharmacy must keep the prescription.
Brazil Pharmacy Stocks: How Far They Fell
Pague Menos closed down 8% at R$3.45 (US$0.67), after falling more than 10% right after the news, InfoMoney reported. RD Saúde, which owns the Raia and Drogasil chains, fell 5.38% to R$18.45 (US$3.56).
That was the second-biggest drop in the Ibovespa index that day. Panvel, a chain based in southern Brazil, lost 4.90% to R$12.43 (US$2.40).
The wider market was weak too, with the Ibovespa down 0.99% on rising US bond yields and fiscal worries at home. The pharmacy falls were far steeper.
What Mercado Livre Announced
The company made the announcement at its Meli Experience event in São Paulo. Its own pharmacy, bought as Farmácia Cuidamos in a deal completed last year, will fill the orders.
Buyers upload a prescription, and a Mercado Livre pharmacist checks it before the order ships. The pharmacy keeps the prescription, as a physical store must.
Fernando Yunes, who heads Mercado Livre in Brazil, said the company had evolved “with responsibility” from over-the-counter medicines. The next step, he added, is a marketplace linking partner pharmacies directly with consumers.
It began with over-the-counter medicines in a São Paulo pilot in March, which showed high search volumes and conversion rates. In July it added prescription drugs that do not require the pharmacy to keep the prescription.
The company already sells medicines in Argentina, Chile, Colombia and Mexico. Brazil is its largest market.
It is also opening a marketplace for pharmacy chains. Three will be on the platform in October, with a target of 20 partner pharmacies in greater São Paulo by year-end.
Those partners will feed a new delivery service, Envios Agora, which promises delivery within 60 minutes. It covers items weighing up to 15 kilograms.
The service is being tested in selected São Paulo neighbourhoods and will be scaled up across the city. The plan is for pharmacy and supermarket items to be part of it.
Mercado Livre also plans 20 new large logistics sites in Brazil by the end of 2026. It already runs more than 3.3 million square metres of logistics space in the country.
In state capitals, 59% of its fast deliveries already arrive by the next day, the company says. In São Paulo state, the figure is 75%.

Why Investors Took Fright
“The weight-loss pen makes it worse, because it brings the best customer a pharmacy has today,” said Cristiano Luersen of Wiser Investimentos. He told InfoMoney that Pague Menos has said these patients spend about nine times more than average.
They also visit the store 50% more often, according to the same figures.
Luersen put it simply: “Losing a sale is one thing. Losing the customer who guaranteed traffic every month is quite another.”
He also argued the sell-off may have gone too far. Mercado Livre started in March and moved again in July, so this was the third step on a ladder in plain view.
In his view, investors were not cutting their profit forecasts in one day. They were cutting what they were willing to pay for those profits, in a moment of fright.
Mercado Livre says it wants to work with pharmacies, not against them. Yunes said the idea was “not to compete with pharmacies, but to collaborate”.
Is It Legal?
Law 15.357 of 20 March 2026 lets licensed pharmacies hire e-commerce platforms for logistics and delivery. Anvisa, the health regulator, has opened a process to write the detailed rules.
In August, Anvisa revoked earlier bans on marketplaces including Shopee, Mercado Livre, iFood, Rappi and Americanas.
Abrafarma, which represents the big pharmacy chains, says it will question the sales at Anvisa. It cites an Anvisa note from 10 August saying revoked rules do not automatically authorise drug sales.
Why It Matters If You Live or Invest in Brazil
If you live in greater São Paulo, it is another way to buy prescription medicines. Delivery can take as little as an hour once partner chains join.
Yunes said demand was already there on the platform but could not be met because of regulation. The most sought-after items include sexual-health products and treatments for nicotine addiction.
For investors, it is a test of how well Brazil’s pharmacy chains can defend their most valuable customers. It also shows how far MercadoLibre, already a big lender through Mercado Pago, wants to reach into daily life.
What Comes Next
Watch how Anvisa responds to Abrafarma and when it publishes final rules for marketplace sales. Those rules will decide how far the model can go.
Mercado Livre plans to add its first three partner chains in October. The size of those chains will show how far the big networks are willing to cooperate.
